Healthy Communities works in partnership with the Healthy Start Coalition of Flagler and Volusia, Inc. to provide screening education and outreach services to providers of clinical prenatal and delivery services in an effort to improve pregnancy and birth outcomes for expectant mothers and babies.
Medical providers receive information about the Healthy Start program and guidance on proper completion of the prenatal and infant screening questionnaires. The simple, one-page prenatal screen is filled out by both the patient and medical personnel at the first prenatal appointment. The infant screen is completed after delivery. Information from the questionnaires can help women access the care, support and services they may need for a healthy pregnancy and a healthy baby.
