Patient Navigator, Lung Cancer

The Patient Navigator at Halifax Health – Cancer Center for Hope is an integral part of your lung cancer treatment team. The Patient Navigator will help guide you through your treatment, assist in organizing your care and provide education and assistance each step of the way. We want to make sure that you get the support and help you need.

The Patient Navigator specializing in lung cancer will empower you by supplying you with current information and education regarding your diagnosis, cancer treatment, management of any side effects of treatment and providing available resources. You will receive the appropriate monitoring and self-care education to ensure the highest quality of life possible during your journey.

Once a referral is made, patient navigation services may include:

  • Assisting and arranging your care
  • Performing a needs evaluation and referrals to appropriate resources
  • Primary contact during your cancer journey

Patient Navigator services are available whether you are an inpatient at Halifax Health or being seen at the Halifax Health – Cancer Center for Hope outpatient clinics.

Remember, you are not alone on this journey. We are here to support you all the way through your treatment.
