To verify affiliation for the provider applying at your facility, we are pleased to announce that Halifax Health now offers verifications of practitioner affiliations online. This service will allow organizations to quickly and easily generate and print a verification letter.

Please use the following information to access our website:

• Address:       www.mdquery.com/halifax
• Login ID:       Halifax
• Password:    query

Once you have logged in, please enter the provider’s last name, first name and the last four digits of the social security number. These fields are case sensitive.
