The community is invited to attend this free event that will provide an overview of Five Wishes – an easy to use legal document that allows adults of all ages to plan how they want to be cared for in case they become seriously ill.

Five Wishes has become America’s most popular living will because it is written in everyday language and helps start and structure important conversations about care in times of serious illness.  It is the first living will to allow users to address their personal, emotional and spiritual needs along with their medical wishes.
