Our Pledge Regarding Your Health Information

We understand that medical information about you and your health is personal. We limit the availability of information to the specific, defined conditions you consent to share. You may request to share or withhold your information at any time. We are committed to protecting medical information about you. This applies to all of the records of your care generated by us, whether made by our personnel or doctors involved in your care.

We create a record of the care and services you receive from us. We need this record to provide you with quality care and to comply with certain legal requirements.

We may disclose medical information about you only for certain purposes:

  • To provide you with medical treatment or other services
  • To organizations that handle organ procurement or organ, eye or tissue transplantation (if you are an organ donor)
  • For worker's compensation or similar programs
  • To someone who helps pay for your care (e.g. an insurance company or a third party).
  • To a friend or family member who is involved in your medical care.
  • In response to a court order, subpoena, warrant or similar request.
  • To an entity assisting in a disaster relief effort so that your family can be notified about your condition, status and location.

Right to Access

You have the right to access medical information that may be used to make decisions about your care.

Right to Amend or Correct

If you feel that medical information we have about you is incorrect or incomplete, you may ask us to amend or correct the information. You have the right to request an amendment for as long as the information is kept by or for us.

Right to an Accounting of Disclosures

You have the right to request an "accounting of disclosures." This is a list of the disclosures we made of medical information about you, other than for treatment, payment or healthcare operations as described above.

Billing and Pricing Information

Pricing information is based on a compilation of charges for the average patient and that each patient's bill may vary from the average depending upon the severity of illness and individual resources consumed. The price of service is negotiable for eligible patients based upon the patient's ability to pay.
