New Patient Appointments

We would like to welcome you to our practice. The Centers for Oncology are part of Halifax Health's comprehensive cancer program. The Centers for Oncology see new patients by referral from another physician. We also welcome the opportunity to provide second opinions. If you have a diagnosis that you feel would benefit from treatment by the Centers for Oncology, please have your primary care physician or referring specialist contact us at 386.254.4061. Our patients are seen by appointment Monday - Friday. If you experience a medical problem that is such that you must be seen immediately, every effort will be made to see you at our office or the hospital emergency room.

Once your physician has called in to make an appointment for you, our referrals area will contact the physician who is providing for your care in an attempt to obtain all pertinent medical history regarding your diagnosis. The referring physician will usually have most of the necessary records. If additional records are needed, you may have to come in and sign a Release of Information consent form which gives us permission to obtain your medical records. If we are still unable to obtain the medical records, it may be necessary for you to contact your physicians directly and make arrangements for them to be sent to us. If arrangements have been made, please contact us 48 hours prior to your first visit to ensure the records have arrived. You can call 386.254.4000, x3952 and speak with one of our medical records specialists to verify delivery. It is important for the physician to have and review these records so that he/she can provide the best possible care for you. Your first appointment with one of our physicians will be approximately 45 - 60 minutes in length. This first visit is longer than a regular follow-up visit to allow the physician the opportunity to thoroughly review your medical records with you and perform a physical examination. If you have any special needs prior to coming in to our facility for your first visit such as a foreign language interpreter, a wheelchair, or any other need, please let us know by calling 386.254.4211.

It will be necessary for you to fill out a Medical History & Medication Summary if you will be seeing one of our medical oncologists or a Radiation Oncology Medical History Form if you are seeing one of our radiation oncologists--and bring it with you to your first visit. We also ask that you fill out and bring with you an Authorization to Release Medical Information Concerning Outpatient Treatment to Family Members for Care and Notification Purposes. This form is needed so that we know who we can give information to regarding your care. All of these documents will be added to your chart. We will also need your insurance cards and a picture ID. If you have a Living Will, Designation of Health Care Surrogate or Durable Power of Attorney, we would like you to bring a copy of those documents for your chart as well. We ask that you arrive at least 30 minutes prior to your appointment so that you will have time to check in with our receptionist and meet with our registration personnel.

Appointments for Established Patients

If you are unable to keep your appointment or need to reschedule an appointment, we ask that you notify our office 24 hours in advance.
