Healthy Communities works in partnership with the Healthy Start Coalition of Flagler and Volusia, Inc. to conduct screening education and outreach services to providers of clinical prenatal and delivery services in an effort to prevent adverse health outcomes such as pre-term labor, low birth weight babies and infant mortality. Providers receive information on the Healthy Start program and guidance on proper completion of the screening questionnaire. There are two opportunities for the expectant mother to do this. The first is offered at the prenatal care provider's office. The second is offered at the hospital after the baby is born.
The simple one-page questionnaire is filled out by the patient and medical personnel. Information from the questionnaire can help patients get the care, support and services they need to have a healthy pregnancy and a healthy baby. Eligible participants are pregnant women who are at risk for poor pregnancy outcomes and infants/children up to age three at risk for poor health or developmental outcomes.
