Monthly cost:
Premium payments depend on household income and family size. Most families pay $15 or $20 per month for all the children in the family. Some are not required to pay a premium. And, families with incomes too high to qualify for these premiums can still enroll by paying the actual cost of the monthly coverage for each enrolled child.
