The Hospice of Volusia/Flagler

The Hospice of Volusia/Flagler, licensed since 1979, has been the community not-for-profit hospice caring for those with life-limiting illnesses and their families. As one of the oldest and largest hospices in the nation, the expertise of the 400 staff members and 600 trained volunteers enables more than 600 patients each day to live every moment to its fullest.

The Hospice of Volusia/Flagler serves all qualified people regardless of race, color, national origin, age, sex, handicap, religion, sexual orientation, disease type or ability to pay. Referrals to hospice can be made by anyone. To be eligible for The Hospice of Volusia/Flagler services, a patient must:

  • Live in Volusia or Flagler county
  • Have a life-limiting diagnosis with a life expectancy of six months or less
  • Have a desire to receive comfort, care and compassion instead of cure

Although hospice care enables most patients to stay at home during their final months of life, hospice services are provided wherever the patient resides – in an assisted living facility, nursing home, hospital, our 16-bed inpatient Care Center in Port Orange, or our 5-bed Hospice House in New Smyrna Beach. Hospice services are covered under Medicare Part A, Medicare HMOs, Florida Medicaid, and most insurance companies. For those without financial resources, a sliding fee scale is available in addition to memorial donations we receive which can help offset necessary care.

Healthy Communities

Since 1993, Healthy Communities has served as an advocate for children’s health in Volusia and Flagler counties. An affiliate of Halifax Health, the 501(c)3 was initially responsible for administering the nationally acclaimed Healthy Kids child health insurance program.

Healthy Communities’ goals include: identifying and enrolling uninsured children in Healthy Kids and Florida KidCare; preventing childhood injuries through water safety instruction, proper use of infant/toddler car seats and booster seats, provision of bike helmets and other initiatives of the Safe Kids Coalition; and, promoting healthy birth outcomes and optimal child development through the Healthy Start program. Overall, Healthy Communities focuses on providing educational and other health-related programs that can positively impact the general health and welfare of all Volusia and Flagler county children and their families.

Florida Health Care Plans

Florida Health Care Plans is the premier health plan in Volusia and Flagler counties serving over 60,000 local residents through a variety of large and small employer based plans, Medicare Advantage Plans and the Healthy Kids Program.

Florida Health Care Plans, which contracts with all local area hospitals, is implementing the latest in electronic health records, electronic communication and e-prescribing to assist its broad network of caring professionals in providing compassionate, high-touch high-tech care to keep those members who are healthy, well; help those with disease risk factors reduce those risks; and help those with serious illness optimize their health outcomes.

Volusia Health Network

Serving Volusia, Flagler and Seminole counties since 1984, Volusia Health Network is the premier provider of quality health plans and health promotion programs in the area.

Volusia Health Network offers Preferred Provider Organization (PPO) and Exclusive Provider Organization (EPO) networks. These networks represent affiliations of physicians, hospitals and other healthcare providers, who form a network to assist self-insured businesses in tailoring health plans to meet the unique needs of their employees. The PPO network includes more than 450 physicians and the EPO network includes more than 700 physicians encompassing all major medical and surgical specialties.
