Halifax Health has collaborated with University of Florida Health to bring advanced endocrinology to Volusia, Flagler, and Brevard Counties. Poonam Kapadia, MD is a Board-Certified Internist with specialty training in Endocrinology, Diabetes and Metabolism.
Nancy and Lowell Lohman, along with their family, are concerned by the increasing expense and education needs of those living with diabetes and are committed to help make an impact to help those in our community.

Poonam Kapadia, M.D., is board-certified in internal medicine and endocrinology, diabetes and metabolism. She completed her bachelor’s degree at Washington University in St. Louis, graduating with honors and a double-major in biology and anthropology. She subsequently earned her medical degree from the Morsani College of Medicine at the University of South Florida and completed her internship in internal medicine at Emory University in Atlanta, Georgia. Dr. Kapadia completed her internal medicine training at the University of Central Florida and then pursued a two-year fellowship in endocrinology, diabetes and metabolism with the University of Florida in Jacksonville.
Dr. Kapadia’s clinical interests include the management of diabetes and thyroid disease, as well as adrenal and pituitary pathology. She is committed to working with her patients to provide them the tools and education necessary to manage their diabetes and minimize complications from this chronic disease. Along with practicing medicine, Dr. Kapadia enjoys trying new recipes, traveling and spending time with her husband and young son.
Book AppointmentAndrea Forster, BS, RN, CCM, CDCES is a Diabetes Nurse Navigator with The Lohman Center for Diabetes & Endocrinology. She assists patients with diabetes by identifying barriers to care and providing education and resources.
Forster attended Roxborough Memorial Hospital School of Nursing in Philadelphia, Pa., and received her Bachelor of Science in Health Care Leadership from National Louis University. As a registered nurse for over 35 years and a certified case manager for over 20 years, her background includes critical care, home health, utilization management, case management and leadership.
Diagnosed with Type 1 diabetes as a child, educating others to manage their diabetes has been a passion of Forster. She became a Certified Diabetes Care and Education Specialist (CDCES), and completed the Diabetes Educator Level 2 program with the American Association of Diabetes Educators. Additionally, she completed the NDPP Lifestyle Coach with the Centers for Disease Control National Diabetes Prevention Program.
The Nurse Navigator specializing in diabetes helps patients stay active and feel better by providing educational and lifestyle coaching. The Nurse Navigator performs screenings and monitors patients throughout treatment with pre-diabetes and diabetes. She Identifies barriers to care and provides education and resources. Halifax Health - Lohman Center for Diabetes and Endocrinology is the only facility in the region to offer this role.
